All accounts are payable within 15 days of statement date. The statement date is the last working day of each month. Accounts not paid within stated terms will be subject to Cash-On-Delivery status and a late charge of 2.0% of the unpaid balance. AIl fees and schedules are subject to change without notice
Dmitriy’s Dental Studios provides a complimentary pick-up service In Los Angeles and North Orange Counties. For all other cases shipping is charged per invoice at $7.50. Please call for rates when shipping to Hawaii or Alaska.
VENEERS & EMPRESS RESTORATIONS 1 -YEAR CONDITIONAL WARRANTY
If fractures occur during the cementation application. Dmitriy’s Dental Studios will remake the restoration at 50% of the original service fee, plus shipping and other incidentals.
PROCERA, LAVA, CAPTEK PFM, & CERAMICS WITH HARD CERAMIC CORE 5-YEAR CONDITIONAL WARRANTY

All warranties are conditional. Patient must receive regular dental recall check-ups from Dmitriy’s Dental Studios clients prac- tice as well as meet all of the post-care recommendations prescribed, including, but not limited to, mandatory night guard compliance. Any damage due to trauma is excluded from warranty: Additional restorative procedures, including, but not limit- ed to, endodontic treatment, addition of a partial denture and loss and/ or failure of teeth, may void warranty. Dmitriy’s Dental Studio is not responsible for charges regarding teeth that receive restorations, including, but not limited to, cases of loss and/or fracturing of abutment teeth prior to, during, or after cementation. This warranty does not apply to any cases and/or restorations that have been recognized as having contradictions by Dmitriy’s Dental Studios. All warranties will be remedied by remake of original class of restorations at no charge. Refunds or credits on accounts are not included in this warranty.
